About the Speakers
Kristy Bernhardt, RN, RAC-CT
Manager of Clinical Analytics
In her current role, Kristy Bernhardt RN RAC-CT manages the MDS department; is the PointClickCare administrator for the facility; and is the developer and collector of data/reporting. She has developed the facility metrics tracking for admission referrals, length of stay, re-hospitalizations, and the like. This new area of Clinical Analytics has developed over the last few years, and is related to being involved in ACO, BPCI, and preferred provider relationships with the surrounding hospitals. Data analysis is what makes Kristy tick – she absolutely loves collecting data and then diving into the numbers to identify areas for quality improvement. She has developed meaningful metrics for use at a local quality council that is helping bridge the gap among acute care providers and post-acute care providers.
Kristy has worked in the long-term care industry for 29 years and counting.
Chuck Czarnik, CHDA
Vice President, Strategic Planning
Chuck Czarnik, CHDA, is Vice President, Strategic Planning at Brookdale, the largest senior housing and post-acute provider in the United States. Chuck is responsible for clinical analytics and informatics strategy, focusing operational reporting, clinical decision support, and population health management. Chuck originally joined Brookdale in IT management, and has been with the company for over sixteen years in various IT, M&A, and operations roles.
Chuck is a member of the HIT Committee of the American Health Care Association (AHCA), and is a member of the HIMSS LTPAC Roundtable. He also serves on the AHIMA CHDA Job Task Analysis Committee and is an AHIMA CHDA exam item writer. He has been involved in numerous industry events, expert panels, and trade publications.
In academics, Chuck studied cognitive psychology, focusing upon statistics, learning technology, and data visualization at Tennessee Technological University (B.S.) and Vanderbilt University (M.S.). He grew up in Franklin, TN, and lives near his hometown with his wife and two children. Chuck is very active in his church’s broadcast video / new media ministry, and seeks adventure, including sport aviation, camping, and mountain biking.

In his current role as Director of Information Technology, Joe is responsible for all of the data and telecom services at Nationwide Healthcare Services locations. Joe is a proud member of the Nationwide Healthcare Services team and works daily on helping everyone provide exemplary care to their residents through the use of technology. Joe joined Nationwide Healthcare Services in his current role about 3 years ago, but has worked with them and other healthcare organizations as a consultant for the past 10 years. Joe works with Nationwide Healthcare Services to roll out the electronic medical records systems, in addition to the rapid expansion of the networks as employee and resident needs have increased the load on the networks.
